Open Enrollment Documents - Indicate EE benefit selections that were done by Proxy

when we look back at EE OE windows, we cannot tell what was done by proxy and what was done by the EE.  It's a great feature, but it needs a little more love.

We'd like to know which benefits were selected by the employee and which were done by using the proxy feature for any open enrollment or OE correction window.

this is useful in general to know, but particularly useful when there are challenges from EE's on what was selected and by who.

We'd like it to record who the proxy was and have that information show up in both EE History Tab and on the pdf attachments that get created and attached to the HR/PR records in Vista.
