We use checklists for a very wide variety of forms - HR status change, discipline, employee financial assistance, equipment inspection, safety reporting, compliance, training content, etc. The columns labeled "Inspector" and "Inspected On" are confusing to our field employees, especially. Could you rename those columns to something more broad/generic, e.g. "Employee Completing Form" or "Completer" or something like that and "Date."

Inspected on is populated from naming one of your checklist header fields "Inspected Date"

I filled out one of our checklists and I don't see anyplace on the form where it asks for "Inspected On". And in our lists of checklists completed, that field is empty. What is that field used for?
And yes, Date should definitely be there! That is one of the first things we need on checklists.
