Our Voluntary Life Insurance benefit allows the employee to elect/decline coverage for self, spouse and each dependent. So need the ability for employee to select benefit option at family member level.
For each election, they can also select insurance amount; range of coverage with increments per employee, spouse and dependent. e.g.dependent aged 15 days to 26 years $5,000 to $20,000 in $5,000 increments.
Cost of benefit is based on aged of family member (rate per $1,000 dependent on age).
