If a performance review is initialized and you add an item to the review template it does not show up on the review, you have to purge the review and initialize it again.

When Performance Reviews are initialized, we lock in the template setup at that specific time for that Performance Review instance. If we allow updates to these after they have been started, it would cause many downstream issues so this is not something we are likely to ever implement.
