Some reviewers ask for coding review or more info but don't reject the invoice. When this happens the AP clerk (responsible person) has no way of knowing it happens unless the reviewer sends an email (which they don't)

This can be done with Vista Workflow notifiers. We have a notifier that goes out. There may be a pre-built one, but they are really easy to build SQL query, add that to Workflows and then use that query in your WF Notifier.
