Currently it seems as though we can only have one custom field per row. It would be very helpful to be able to have at least two fields per row. We are trying to build our temperature screening forms through check lists but it's challenging trying to have a form that can cleanly cover multiple employees.

This is similar to suggestion KDS-I-419 - in essence, having a table with multiple columns for the data and the ability to add more rows as needed.
