It would be great if you could have a fillable form (or a Keystyle created form) that employees could fill out and it would attach as a PDF to the employee record. This would be great for collecting important information that doesnt currently have the appropriate fields in Viewpoint. One example, our company has to keep Life Insurance beneficiaries on file for each employee. The "beneficiary" fields in Viewpoint are MUCH too limited for our needs. It would be great if we could create a form in Keystyle, send out to employees and track the responses. The form would just be attached as a PDF file to the HR Resource Master Record (similar to the Onboarding Forms).

Is there an update on this functionality?
I have a work around currently outside of Vista with links in the company announcements. I would prefer to have this as a PDF and saved in a table in Vista that could be used as a report.
We just went through Benefits Enrollment in the Portal (7/1/21 renewal). We had employees update their benefit dependents but we were unable to separately capture the employee Life Insurance beneficiaries. Now we have to figure out how to communicate and collect Life Insurance beneficiaries separate from our Benefit Enrollment. Let's speed up this needed function!

It would be great if you could define it not only to HR but have the type as an option too. This would be so beneficial to us too!
This would be a great feature throughout the Portal. Communicated to the employee, have them complete a fillable form, and submit back through the Portal.
