It would be useful to allow the template title to appear on the form for both Self Review and Manager Review so that if you have multiple templates they are easily ordered and managed, rather than appearing entirely similar.

The ability to add or customize your Performance Review headers already exists today. When editing a template, enter the header you would like in the Edit Header Text section.

Attached are some screenshots of the Template name in the Manage Template section and the desired location on the review form that is launched. We would like this to appear in all stages of the review process. We use multiple templates for many different business units so it is important to be able to differentiate them.
